1. 1. Download and fill out this form completely Myanmar e-Visa Form and save it to your computer .
   2. Scan the following documents for all e-visa types:
      • A color scan of the traveler’s passport information/photo page – 10KB – 300 KB PDF format.
      • One recent passport style photo. In color, no glasses, neutral expression 10KB – 1MB – 2” x 2” jpg format
      • Non-US citizens must also submit a copy of their Permanent Resident Card or U.S. Visa and I-94.
        Travelers may print the Most Recent I-94 Form from https://i94.cbp.dhs.gov/.
      • For Business Travelers Only:
        • A scan copy of Letter of Invitation from the company to be visited (The letter should be addressed to
          the applicant and include job title, company name and address and purpose of the visit. The letter
          should also be printed on a letterhead, signed by a representative of the company.).
        • A Copy of the host company’s business registration.
        • A Hotel Reservation showing your name and for the duration of the stay.

3. 3. Either email all documents to e-visa@passportvisaadvisors.com or securely upload by using the upload box below.

Documents Naming: Please help us quickly identify and retrieve your documents by naming them with Last Name first followed by document type, with no comma in between e.g. SmithBirthCertificate, SmithTravelItinerary, SmithPhoto.
