1. Download and fill out this form completely Brazil e-Visa Form and save it to your computer .
2. Scan the following documents for all e-visa types:

• • A color scan of the traveler’s passport information/photo page – 10KB – 300 KB PDF format.
  • One recent passport style photo. In color, no glasses, neutral expression 10KB – 1MB – 2” x 2” jpg format.
  • A Confirmed Flight Itinerary with your name and entry and departure country.
  • A Hotel Reservation for the duration of the stay.
  • A scan of applicant’s recent bank statement (minimum 2000USD) or most recent paycheck.
  • Business Applicants:
    • Must provide a signed employer letter and signed invitation letter from the company/sponsor in Brazil.
  • Minor Applicants (Under 18):
    • Must provide a scan of parent or guardian’s national ID.
    • a scan of the parent/guardian’s Bank statement (min. 2000USD).
    • a scan of applicant’s long form birth certificate showing parental relationship or court order showing guardianship.

3. Either email all documents to e-visa@passportvisaadvisors.com or securely upload by using the upload box below.

Documents Naming: Please help us quickly identify and retrieve your documents by naming them with Last Name first followed by document type, with no comma in between e.g. SmithBirthCertificate, SmithTravelItinerary, SmithPhoto.
