1. Download and fill out this form completely Brazil e-Visa Form and save it to your computer .
2. Scan the following documents for all e-visa types:

• • A color scan of the traveler’s passport in PDF format
  • One recent passport style photo less than six months old in color, no glasses, neutral expression, in jpg format
  • Round Trip Flight Itinerary or Trip Sheet
  • A scan of applicant’s recent bank statement (minimum 2000USD) or most recent paycheck
  • Business Applicants: Must provide a signed employer letter and signed invitation letter from the company/sponsor in Brazil
  • Minor Applicants (Under 18): Must provide a scan of parent or guardian’s national ID, a scan of the parent/guardian’s
    Bank statement (min. 2000USD) & scan of applicant’s long form birth certificate showing parental relationship or court
    order showing guardianship

Documents Naming: Please help us quickly identify and retrieve your documents by naming them with Last Name first followed by document type, with no comma in between e.g. SmithBirthCertificate, SmithTravelItinerary, SmithPhoto.
