1. 1. Download and fill out this form completely Pakistan e-Visa Form and save it to your computer .
   2. Scan the following documents for all e-visa types:
      • A color scan of the traveler’s passport information/photo page.
      • One digital passport style photo. In color, no glasses, neutral expression 10KB – 1MB – 2” x 2” jpg format
      • A Confirmed Flight Itinerary with your name and entry and departure country.
      • A Hotel Reservation for the duration of the stay.

Documents Naming: Please help us quickly identify and retrieve your documents by naming them with Last Name first followed by document type, with no comma in between e.g. SmithBirthCertificate, SmithTravelItinerary, SmithPhoto.
